Dr. Abigail Donate-Peralta

Dr. Abigail Donate-Peralta (Dr. Abi) is a bilingual psychologist with seven years of experience in the mental health field. She specializes in educational psychology and neurodevelopmental disorders, and provides comprehensive neurodevelopmental, neurocognitive, and social-emotional evaluations in both English and Spanish at Rooted Behavioral Health. Dr. Abi is passionate about supporting individuals across all age groups, helping them better understand their strengths and needs. Outside of her professional work, she enjoys traveling, spending time with family,  and listening to podcasts

Dr. Carmen Plasencia

Dr. Carmen Plasencia is a Psychologist Candidate who has worked in the community mental health space for four years. She fell in love with the practice of psychological assessments when she realized the positive impact they can have on client’s lives by helping them better understand themselves and by removing barriers to care. Although Dr. Plasencia sees all ages, she has a special passion for working with teens and adults who identify as neurodivergent, multicultural, and/or who are survivors of trauma. At Rooted Behavioral Health, Dr. Plasencia conducts neuropsychological, neurodevelopmental, and socioemotional evaluations in both English and Spanish from a culturally humble and trauma-informed approach. When she’s not doing what she loves at work, Dr. Plasencia enjoys trying any new delicious foods she can get her hands on, getting lost in a good book, and spending time with her partner and beagle.

Dr. Kirsten Kloock

Dr. Kirsten Kloock is a Licensed Psychologist and has worked as a clinician for 18 years. Dr. Kloock decided to specialize in psychological testing during her postdoctoral residency, finding the work to solve the puzzle of the mind fulfilling.

 

She loves to see hope on clients’ faces after skillful and thorough empirically based evaluation helps to illuminate the path forward. Dr. Kloock has experience with clients throughout the lifespan and a variety of different diagnoses, and particularly enjoys working with neurodivergent individuals. She performs neuropsychological, neurodevelopmental, and socioemotional evaluations at Rooted Behavioral Health.

 

In her spare time, Dr. Kloock enjoys taking in the outdoors in beautiful Colorado.

Jonah Freed

Jonah Freed is a Licensed Clinical Social Worker with Rooted Behavioral Health. He has worked in various settings supporting mental health over the last seven years and also served in the Peace Corps in Peru. Jonah believes therapy can be the catalyst to a more meaningful life and feels grateful to work at Rooted Behavioral Health because of the opportunity to support growth through a healthy connection. He provides psychotherapy by telehealth and in-person in English and Spanish with children (ages six and older), teens, and adults, working collaboratively with clients using behavioral, cognitive, mindfulness, third-wave, psychodynamic, trauma, and play therapies to support change and healing.

Sheila Stanley, Psy.D

Sheila Stanley, Psy.D. is a licensed clinical psychologist and Testing Psychologist at Jefferson Center. She earned her doctorate from Nova Southeastern University in 2020, where she completed a concentration in forensic psychology, followed by internship at Springfield Hospital Center and postdoctoral training with the Boulder RISE Program. Her strong foundation in forensic psychology continues to shape her trauma-informed, systems-aware approach, with her work in community mental health settings deepening her focus on culturally responsive assessment, supervision, consultation, and program development. She enjoys the mutual learning that comes with supervising interns and supervises the Supervision of Supervision rotation. Outside of work, she lives with her husband and two cats, and loves catching live music whenever she can.

Dr. Adam Maher

Dr. Adam Maher is a Licensed Psychologist with the psychological testing team. He has worked for the Jefferson Center for Mental Health for seven years and has over 17 years of experience working in the mental health field. Dr. Maher is a Colorado native and loves getting outside with his wife and two children. At Rooted Behavioral Health, Dr. Maher enjoys seeing all ages for psychological testing. He is passionate about finding the uniqueness of every individual and helping them reach their potential. He regularly performs ADHD, Autism, socioemotional and court ordered evaluations.

  • Our new office is currently under a re-fresh and will be located at 7828 Vance Drive, Arvada, CO 80003. Until then, we are seeing therapy clients via telehealth and psych assessment clients at 4851 Independence St, Wheat Ridge, CO 80033.

What can a psych assessment diagnose?
